How to submit supporting materials

  1. See the table below for a list of required forms and materials for each award.
  2. Nominees can access the required forms below and upload them to their UW Google G Suite nomination folder. Links to nominee folders will be sent on December 7, 2020.
    Please note: Completed nomination summary forms and checklists should be uploaded to the nominee’s general nomination folder, all other materials should be uploaded to their respective subfolders within the nominee’s general  folder. 
  3. Supporting letters and materials must be in 12-point font.
  4. Files must be uploaded to the nominee’s UW Google G Suite folder by 5:00 p.m. on Wednesday, January 20, 2021. We can not accept late files, individually submitted items, or hard copies.
